It sounds like she's fairly against SSRIs (“medicated to numbness” is a common thing people who are opposed to them say), but her MD could do a lot with other medications. OP might have more luck with a strategy of tackling the physical pain she's in first, then working on the emotional stuff. No one is mentally at their best when they can't sleep and are hurting.
