She's 18 years old. She never got in the habit of putting on a fresh pair every day. She isn't in the habit of showering every day. She's young.
If it doesn't really bother you all that much, let her take care of herself for now. If it does bother you, tell her gently, subtly what it is that bothers you about it. Don't tell her how to online her life.

I think the difference refers to whether it’s medically necessary. Like if there’s a tumor or illness that requires tubes/ovaries/uterus to be removed vs me just wanting to never get pregnant.

The dynamic completely changes if she’s in school or a stay at home girlfriend. Then low key, she’s a sugar baby and it then explains why she’s doing so much to convince him to marry her rather than breaking up with him. If she’s never had a job and she can’t take care of herself independently then he’s her meal ticket, and he knows it!
